Job Requirements Must have professional license/certification verification as required for the position. Must have completed Prime Core Competencies Exam with a score of 80% or higher. Must have completed Prime Skills Checklist. Must have completed Prime Specialty Exam with a score of 80% or higher. Must have Prime Education Verification (diploma or transcripts). Must have a clear Physical/Health Statement completed within 60 days of start, stating clear to work. Must have a National Criminal Background Check within one year of start date. Must complete Prime Agency Attestation with up-to-date information. Must complete Prime SAM/EPLS search for excluded individuals. Must complete Prime Orientation Packet including "Essentials" Form and Post Test. Must have signed Prime Job Description from within the year. Must have Prime Form I-9 (Employment eligibility verification) or agency attestation. Must have Driver's License, State ID, or Passport. Schedule Information Shift from 07:00 to 17:30. Additional Details Annual Respirator Fit Test is optional but recommended. COVID-19 vaccine with booster required or declination completed. Proof of MMR vaccination or positive titer required. Record of Hepatitis B immunizations, immune titer, or declination accepted. Influenza vaccine or declination required between November 1st and March 31st. TB test within one year of start date required. Proof of Varicella vaccination or positive titer required. Tdap vaccination required; declinations not accepted at some facilities. Positive PPD requires negative chest x-ray within 3 months. Prime Color-blind test must be completed annually. Prime Badge Photo is optional and facility specific. Prime Drug Screening (minimum 10-panel test including THC) within 60 days of start required. Name Verification Affidavit is optional for name discrepancies.
