CenterWell Senior Primary Care - - Responsibilities: Provide direct patient care to patient under the direction of the RN and according to the Aide Plan of Care; Take accurate temperature, pulse and blood pressure measurements and report abnormal results to supervisor; Assist with personal hygiene and activities of daily living including bathing, grooming, and oral care; Assist in feeding patients and monitor nutrition and fluid intake; Document interactions with patients and other staff to ensure continuity of care
